Abstract

This study reported the repair of a circumscribed periapical lesion with a diagnosis suggestive of chronic periapical periodontitis. A 49-year-old male patient was referred for endodontic treatment of the lower premolar. Clinically, an active fistula, caries, and a negative response to cold thermal stimuli were observed. Radiographically, extensive caries and periapical radiolucency were observed. After anesthesia and exploration of the canal with a number 10 K-File and simultaneous irrigation with 2.5% Sodium Hypochlorite, the actual tooth length was determined using an apex locator. Root canal preparation was performed with a reciprocating V File TDK® #40.06. Subsequently, an intracanal medication (BIO-C Temp) was applied for 30 days, which was renewed for another 30 days. Obturation of the root canal system was performed using the single cone technique combined with BIO-C Sealer cement. Clinical and radiographic follow-up was conducted 12 months after canal obturation, showing repair of the periapical lesion. It is concluded that endodontic treatment combined with intracanal medication resulted in clinical and radiographic success, as repair of the periapical bone radiolucency occurred.
