HEART FAILURE WITH REDUCED EJECTION FRACTION AND CONGESTIVE LIVER DISEASE IN A 28-YEAR-OLD MAN - CASE REPORT

Abstract

INTRODUCTION: Heart failure is characterized by the structural and functional inability of the ventricles to fill and eject, and is divided into preserved ejection heart failure and reduced ejection fraction heart failure. Congestive liver disease is caused by impaired hepatic blood flow or increased pressure in the liver and is often correlated with congestive heart failure. EXPERIENCE REPORT: A 28-year-old man with a history of dyspnea on exertion that began 60 days prior, progressing to symptomatic worsening and dyspnea on minimal exertion (NYHA 4), orthopnea, and paroxysmal nocturnal dyspnea. The patient had complaints of epigastric pain and nausea for more than two weeks, associated with watery diarrhea and dark urine. Echocardiography and laboratory tests (AST/SGOT, ALT/SGPT) and abdominal ultrasound were performed. DISCUSSION: Heart failure and liver failure are coexisting diseases, as both are affected by systemic disorders that interfere with both organs. (4) Furthermore, HF is responsible for causing acute and chronic liver damage, due to ischemia and blood congestion. The patient's history of obesity may have influenced the early manifestation of heart failure, since obesity, overweight, and abdominal obesity are currently considered independent risk factors for the onset of heart failure. CONCLUSION: The relationship between heart failure and congestive liver disease is frequent and should be considered in the clinical approach to patients.
