Abstract

Patient-centered care has become a key care model for improving quality in high-complexity hospital settings, particularly in contexts marked by care fragmentation, multiple professional involvement, and high clinical complexity. This study aimed to analyze multiprofessional strategies related to care integration, interprofessional communication, and continuity of care in hospitals, from a patient-centered care perspective. A narrative literature review was conducted using searches in the BVS, LILACS, SciELO, PubMed databases, and institutional documents from the World Health Organization, covering the period from 2015 to 2025. After applying inclusion and exclusion criteria, 12 studies were selected for analysis. The findings indicate that integrated multiprofessional practice, combined with structured communication processes and effective care transition planning, significantly contributes to improving care quality and patient safety. Continuity of care and active patient and family participation emerged as central elements in the implementation of patient-centered care. It is concluded that patient-centered care in high-complexity hospitals depends on strengthening multiprofessional collaboration, qualified communication, and institutional strategies aimed at ensuring continuity of care, resulting in safer, more humanized, and needs-oriented healthcare delivery.
